Stronger links to SNFs reduce 30-day readmission rate

The relationship between hospitals and skilled nursing facilities (SNFs) in reducing the 30-day readmission rate was analyzed in this large-scale study of 2.8 million patients discharged between 2004 and 2006.  It was found that stronger relationships between hospitals and SNFs led to reduced rehospitalization.  This relationship held for hospitals that owned the SNF, but was also found between hospitals and SNFs that they did not own.  The findings lend support to concentrating discharges in particular SNFs.
