Top readmission prevention strategies: home health partnership is number one according to 2013 survey

The following results are based on a “buzz survey” of executives at health care provider organizations, primary hospitals and health systems.  There were 106 responses.

Which Strategies Are Key to Reducing Preventable Readmissions?  (The top three answers):

  • Partnering with home health providers (73 percent)
  • Scheduling follow-up visits with physicians (69 percent)
  • Partnering with long term care providers (64 percent)
