POPULATION HEALTH: mental health and primary care

Changes in the health care delivery system prompted by the health reform legislation include a heightened interest in collaboration and clinical integration.  This article is a brief interview with Wayne Katon, MD, a psychiatrist, who has been a long-time advocate of integrating psychiatric care with primary care.  Dr. Katon describes a collaborative care model that relies on a care manager, a nurse or collaborative care professional, to manage the day-to-day operation of a population health management system.  The article includes a schematic showing the relationships between patient, care manager, primary care physician, and psychiatrist.  Places where this model has been implemented are mentioned.
