What are health care HOTSPOTS and SUPER USERS?

There is likely to be a residential building or housing complex in your town that generates very high health care utilization rates — inpatient admissions, readmissions, and emergency department visits.  This is a health care hotspot, but to find these locations, you need all-payer data on all the hospitals in town.  The Camden Coalition of Healthcare Providers has such data about the Camden, NJ, market going back 10 years, which gets real-time input from the three hospitals.   The Coalition identifies health care “super users,” individuals with high inpatient utilization rates and assigns them to high-risk teams or intermediate-risk teams for posthospital follow-up services.  The Coalition has also identified two buildings in the city where patients with high utilization of inpatient care live.  In one of those buildings, a clinic was opened.
