These cited statistics come from this white paper: Goodman, D.C., Fisher, E.S., and Chang, C-H.  After Hospitalization: A Dartmouth Atlas Report on Post- Acute Care for Medicare Beneficiaries.  The Dartmouth Institute for Health Policy & Clinical Practice, Sept. 28, 2011.  Click here for the full text: http://www.dartmouthatlas.org/downloads/reports/Post_discharge_events_092811.pdf  Posted by AHA Resource Center, (312) 422-2050, rc@aha.org
