30-day readmission rates: national, state, county benchmarks

Here are the US average 30-day readmission rates for Medicare patients in 2009:

  • 16.1% for medical discharges
  • 12.7% for surgical discharges
  • 21.2% for CHF (congestive heart failure) patients
  • 18.5% for AMI (heart attack) patients
  • 15.3% for Pneumonia patients
  • 14.5% for Hip fracture patients
